Certificates of Service

Certificate of Service

The Office of Vital Statistics, Ohio Department of Health is seeing a concerning decline in the filing of certificates of service.

We would like to remind everyone that a "certificate of service" form is required by law to be filed for each body brought into Ohio for burial or other disposition. Similar to "certificate of death", the "certificate of service" must be completed in Electronic Death Registration System (EDRS), printed, signed by the funeral director in charge of disposition. Once completed it can be folded and mailed to the LHD for filing as necessary.

Additionally our office has been researching occurrences in which a certificate of service was not filed with our office. Therefore you may receive notification from our help desk for a specific decedent buried in Ohio with your funeral home.
