Leveraging Grocery Stores to Deliver Personalized Nutrition: An Interdisciplinary Model of Care in the Community

The COVID-19 pandemic put unprecedented strain on patients’ health and well-being this past year. If you’ve wished there was a way to help a patient experiencing food insecurity, or a patient struggling to manage their health, there are resources available to you.

The American Heart Association, with support from Kroger Health, created a free online continuing education (CE) training to highlight how collaboration with a retail registered dietitian to deliver accessible, personalized nutrition benefits both patients and health care providers, including pharmacists practicing at the top of their license.
