2011 APPLICATION FOR SCHOLASTIC ASSISTANCE

In order to qualify for an award from the OFDA Scholastic Assistance Committee, the 2011 SAC Application (completed in full), an official transcript, and three letters of recommendation (one must be from a licensed funeral service professional) must be submitted to the OFDA office: P.O. Box 21760, Columbus, OH  43221-0760 no later than July 7, 2011.

Our committee will review each application with equal consideration and select the most qualified applicants for a personal interview with the entire committee.  Those applicants will be notified by telephone or e-mail to schedule a personal interview.  The interviews will take place at the OFDA headquarters, located at 2501 North Star Road, Columbus on Thursday, July 21, 2011. 

All applicants will be notified of the committee's decision.
