AWARDS NOMINATIONS
We strongly encourage all of you to take a moment and look at the PT's, PTA's, and students you work with, those you know through the association, and those you know personally. Consider nominating one or all of them for one of the awards listed below. This is a great opportunity to recognize those therapists who go above and beyond for their patients, for their colleagues, and for our profession. This is a special way to thank these outstanding individuals for all of their hard work and dedication. But we need your help in identifying these individuals!

OUTSTANDING STUDENT AWARD
To acknowledge a Student Physical Therapist or Student Physical Therapist Assistant who has made significant contributions to the professional organization or to the promotion of physical therapy in the community.
EMERGING LEADER AWARD
To identify and honor one Physical Therapist/Physical Therapist Assistant "emerging leader" within the Ohio Chapter of the APTA who has demonstrated extraordinary service early in their career. The individual should have made exceptional overall accomplishments and contributions to the APTA, the state, and the profession to advance APTA's vision.
OUTSTANDING PHYSICAL THERAPIST ASSISTANT AWARD
To acknowledge a Physical Therapist Assistant who has made significant contributions to the profession of physical therapy in the state of Ohio in one or more of the following areas: Clinical Service, Education, Research, Administration, Public Service, Contributions to the Organization.
OUTSTANDING PHYSICAL THERAPIST AWARD
To acknowledge a Physical Therapist who has made significant contributions to the profession of physical therapy in the state of Ohio in one or more of the following areas: Clinical Service, Education, Research, Administration, Public Service, or Contributions to the Organization.
OUSTANDING CLINICAL EDUCATOR AWARD
To acknowledge a clinical instructor for outstanding contributions to the clinical education aspect of the physical therapy profession; To encourage clinical instructors to become credentialed by the APTA and remain active in the physical therapy education community.
MERITORIOUS SERVICE AWARD
To formally recognize the effort and contributions of an Ohio Physical Therapist OR Physical Therapist Assistant to the Ohio Physical Therapy Association during the preceding year (s)
