Position Description: The Nurse Practitioner of Hospice & Palliative Care of Greater Wayne County is responsible to the Clinical Program Director for the coordination and delivery of hospice & palliative care services. The Nurse Practitioner serves as a member of the hospice interdisciplinary team and provides expertise in the area of pain and symptom management. He/She also serves as a liaison and resource to the medical community, promotes effective working relationships with other community providers, and is an advocate for patients served by the organization.
Major Responsibilities:
- Work with inpatient facilities to identify collaborative opportunities and develop appropriate hospice & palliative care programs.
- Function independently and/or as a member of an interdisciplinary team to plan, deliver, and evaluate health care for hospice and palliative care patients.
- Perform patient assessments and other medical procedures, including the prescription of medications consistent with the collaborative practice agreement and conforming to state and federal law at all times.
- Provide hospice & palliative care education to external agencies, community, and other health care providers; develops educational tools in response to identified needs.
- Participate in a schedule of coverage rotation with physicians to ensure access to services and continuity of care consistent with policies and procedures.
- Provide education and serve as a resource for hospice nursing staff and other members of the interdisciplinary team as requested.
- Participate in professional nursing activities with the organization and with other community providers serving on agency committees/team as appropriate.
Position Requirements:
- Current license to practice nursing in the State of Ohio.
- Current license as a Certified Nurse Practitioner in the State of Ohio.
- MSN required.
- Demonstrated experience in area of clinical practice required with a minimum 3 years recent clinical experience.
- Certification in Basic Cardiac Life Support (BCLS) or certification within 3 months of hire.
- Excellent Communication skills, both verbal and written.
- Valid Ohio driver’s license.
- Ability to provide reliable transportation, own auto insurance, and professional liability insurance.
- Favorable references.
