Please consider this a reminder of the August 31, 2015 deadline to submit your organization’s FY15 Safety Plan Summary Form (SH-2) for the July 1, 2014 – June 30, 2015 program year.

 The SH-2 deadline was changed from mid-May to August 31 as a result of BWC’s move to prospective billing.

The SH-2 and supportive documentation should be submitted to safetygrouprating@bwc.state.oh.us.
