NCDA is soliciting any form or document career development professionals might use such as

  • intake forms

  • informed consent

  • employment/internship forms

  • release of information forms

  • policies and procedures.

 

What will NCDA do with the selected documents?

NCDA will post these documents online and members will be able to access them for use in their work.

 

Who can submit documents for inclusion?

Any NCDA member can submit documents for consideration. Submitted documents should be the original work of the contributing member or the member’s organization. Please do not submit materials copyrighted by other individuals or organizations.

 

When do I complete the NCDA Permission to Use Agreement?

Contributors must sign the NCDA Sample Documents Permission to Use Form and submit the agreement with their sample document or form.

 

Who will review the document to determine inclusion?

The NCDA Ethics Committee or a sub-group of the committee will review documents.

 

How will documents be selected for inclusion?

Not every sample form or document will be included in the online repository. Submitted forms or documents will be reviewed by the NCDA Ethics Committee according to the following criteria:

  • Code of Ethics: The content of the sample document or form will be consistent with the NCDA Code of Ethics.
  • Ease of reading: The sample document or form will be well organized and written in clear language with consistent use of graphical emphases.
  • Intended audience: The intended audience(s) will be clearly specified or readily recognized.
  • Scope: The content of the sample document or form will be related to the provision of career services, something essential to assisting clients with the career development process. The document will include the necessary information in relationship to the scope or purpose of the form as well as the intended audience.
  • Conciseness: The sample document or form will be designed to gather information that informs the work between career professionals and clients, without obvious extraneous items.
  • Comprehensive: The sample document or form will be comprehensive relative to the apparent objective.
  • Permission to use: The person submitting the form must be the author of the form or have the authority to grant permission to use it as a sample.

 

Will NCDA notify me if my submission is selected?

Contributing members will be notified regarding the NCDA Ethics Committee’s decision to include their document in the online repository within 4 – 6 weeks of their submission.

 

How long will documents be included in the online repository?

All documents will be reviewed on a bi-annual basis. Those that no longer fit the criteria mentioned above will be removed from the website. Contributing members may be contacted by the NCDA Ethics Committee to inquire about updated versions of their documents.

 

Will the document be modified?

No, the documents submitted will be converted to a PDF (protected digital file). Please submit your documents in the form in which you’d like them to be displayed. If you do not want your organization identified, please remove references to the institution before submission.

 

What if I want to submit another document in the future?

At this time we are accepting forms on an ongoing basis. Future submissions may be emailed to sampledocuments@ncda.org. Please also submit the Sample Documents Permission to Use Form with each subsequent form.
