Purpose: The purpose of this event is to provide education for healthcare professionals and Veteran-focused community on the unique needs of caring for our Veterans at end-of-life.
Who Should Attend: All Health Care Professionals, Nurses, Social Workers, Physicians, Chaplains in Community Hospices, Interdisciplinary Team Members, Discharge Planners in Hospitals & Clinics, VAMCs, and Staff of Veteran Homes, Home Care, Extended Care Facilities, Assisted Living Facilities, Nursing Homes, and all Interested Veterans.
Cost: $45 per person Fee includes all materials, continental breakfast, lunch and CEs/Contact Hours.
