Purpose(s)

Part I: January 18
To gain a general understanding of the basics of of the OASIS C derived Home Health Quality Measures as well as the process measures.  

Part II: February 15
Understand the Outcome Based Quality Improvement report and how it is utilized.

Part III: March 21
To select a specific process quality measure and develop a Plan of Action to improve rates of compliance with best care practice.

Objectives

Part 1: January 18

  1. Describe the background of process measures.
  2. Identify the process measures.
  3. Describe the purposes for process measures.


Part II: February 15

  1. Describe the definition for each process measures.
  2. Identify the elements of the Process Quality Measure Report.
  3. Interpret the Process Quality Measure Report.


Part III: March 21

  1. Describe the selection of the Process Quality Measures for investigation.
  2. Describe the process to determine potential causes for the low rates of compliance.  
  3. Describe the process for developing/ implementing a Plan of Action.
